Intake Forms (to be completed by the first session):

Please fill out the forms (consent, information form, questionnaire and credit card information sheet if you plan to pay with a credit card) either through our super easy client portal here:

or by printing out, completing and providing us with the forms below that pertain to you:

If you would like to use a credit card to pay for your sessions, please print and fill out the credit card authorization form.

If you would like your therapist to coordinate care with another provider (for example, your child's school or teacher, your psychiatrist, primary care physician, etc.), complete this form to authorize release of psychotherapy information:

To help you know what you are looking for on Sepulveda,  Illusions Spa on the first floor of the building facing Sepulveda.  It is the building between the UCLA building and the shopping complex that houses Rubios, Grow the Produce Shop and Manhattan Bagel.

When you park, you want to go up the staircase in the front part of the building (shown above).  Our offices are on the second floor above Illusions Salon.

During the First Session:

If you do not get a chance to fill out the forms before arriving, no worries, we will have the forms ready for you to fill out.  At the beginning of the session, your therapist will go over limits of confidentiality and consent for treatment and answer any questions you have about therapy, our policies or logistics.

Following that, we want to give you the space to talk about what brings you to therapy.  Some people have a lot to say at first, some people have no idea where to start.  Anywhere you are on the spectrum is perfect.  Your therapist will help get what you need out of the first session.

At the end of the session, your therapist will take the last couple of minutes to check in with you about your experience of the first session so that you can have a chance to provide any feedback about what you might need or what worked for you and collaborate with you on a plan for next steps.

It is important that you provide feedback whenever you have it so that you can get the most out of your investment in therapy.  If you find that the therapist you met with might not be a perfect fit, please let us know by emailing adrianwalkerhall@gmail.com.  We are happy to provide referrals or connect you with another therapist in our practice that might be a better match.  We want you to get the support you need in the way you need it.

It is important to understand that it will take some time for there to be trust in the therapeutic relationship (that is the relationship between you and your therapist), for your therapist to get a sense of you and understand the context of what is bringing you or your family to treatment.  We understand that people frequently come to see us when they are in distress and are looking for relief and some answers.  We want you to know that sometimes that happens right away and, sometimes, it can take some time.  Keeping your therapist in the loop about your experience will help.
